Occupational Therapist/COTA

Description

The Occupational Therapist provides services to any district child when determined by IEP. Services include:

• Procurement of necessary physician’s statements (forms).
• Evaluate students referred for consideration of the need for occupational therapy.
• Participate on the IEP team as assigned, including the preparation of timely and appropriate individual IEP team member reports.
• Assist in the development and implementation of the individual education plan as requested.
• Provide direct occupational therapy/consultation.
• Provide training (including in-service) as necessary for significant others, i.e., teachers, parents, etc.
• Provide communication and follow-up with medical personnel.
• Assist in the completion of necessary documentation for the purpose of third party billing.
• Complete necessary local, state, and CESA forms, i.e., membership forms, in a timely fashion.
• Supervision of certified occupational therapy assistant (COTA).
