Description
The Physical Therapist/Certified Physical Therapy Assistant (CPTA) provides services to any district child when determined by an Individualized Education Program (IEP). Services include:
- Procurement of necessary physician’s statements (forms).
- Evaluate students referred for consideration of the need for physical therapy.
- Participate on the IEP team as assigned, including the preparation of timely and appropriate individual IEP team member reports.
- Assist in the development and implementation of the individual education plan as requested.
- Provide direct physical therapy/consultation.
- Provide training (including inservices) as necessary for significant others, i.e., teachers, parents, etc.
- Provide communication and follow-up with medical personnel.
- Assist in the completion of necessary documentation for the purpose of third party billing.
- Complete necessary local, state, and CESA forms, i.e., membership forms.
- Supervision of CPTA.
